Nursing Assistants work as caregivers in all types of health care facilities and agencies, but predominantly in long term care under the supervision of nurses. In order to be admitted into Highland’s program, prospective students much complete the following Illinois Department of Public Health (IDPH) background check form and submit the Fingerprint Form below.
CNA Background Check and Fingerprinting Form
The CNA program requires applicants for the program to submit an Illinois Department of Health background check, as well as a fingerprint release form.
The Background Check and Fingerprinting Form will be forwarded to Highland faculty for review.
